  • Complete the on-line portion

    This includes:

    1. Registering and completing the on-line portion of the Application.
    2. Printing the on-line application Signature Page to include with your Application package.

    You will then be directed to return to this page to complete the following:

  • Pay the application processing fee

    A $50 Application processing fee is required to complete your application. Go to your SUNY Downstate application checklist where you may pay your $50 Application processing fee with your Visa or MasterCard.

    If you wish to pay by check (USA bank checks, postal money order in USA currency) your $50 non-refundable check must be made payable to “SUNY.” Mail your check together with your On-Line Application supporting documents, to the address below.

  • Review and sign the Application Cover Sheet

    Download the Application Cover Sheet

    Use as a checklist to assure that you have completed all steps of the application process.

  • Download and complete the following forms

    1. Letter of Recommendation (2 letters required)
    2. If you answered 'yes' to having been enrolled in EOP/SEEK/HEOP/College Discovery:
      Educational Opportunity Program (EOP) Verification Form
  • Submit the following information

    1. Documentation of 225 hours of Healthcare Experience on Institutional Letterhead and signed by a direct supervisor or aninstitutional representative.
    2. Documentation of 150 hours of Non-Clinical Volunteer Experience on Institutional Letterhead and signed by a direct supervisor or an institutional representative.

  • A complete, self-administered application package is required for admission to all programs. A complete application includes all of the items listed on the Application Cover Sheet (Step 4 above). The entire application is required from you in one envelope at one time.
  • Neatness and legibility will speed the processing of your application.
  • Type or print your name at the top of any separate sheet of paper enclosed with your application.
  • We rely on email as our primary means of communication with applicants.It is essential that you provide a clearly written, valid email account on your application form and keep us updated if you change your email and/or your postal mailing address.
