School of Public Health Application

Applications will be accepted on a space availability basis

  • Instructions

    Please click on the program for which you are applying for to review the Application Instructions and requirements prior to completing an on-line Application.

  • Complete the on-line portion

    This includes:

    1. Registering and completing the on-line portion of the Application.
    2. Printing the on-line application Signature Page to include with your Application package.

    You will then be directed to return to this page to complete the following:

  • Download, review and sign the Application Cover Sheet specific to the program to which you are applying:

  • Download and complete the following forms

    1. Additional Information Form
    2. Recommendation letter — MPH applicants must submit 2 letters, MD/MPH applicants must submit 1 letter, and DrPH applicants must submit 3 letters.
  • Personal statement

    Please explain your reason(s) for pursuing the degree in a particular area of interest and hope to achieve with that degree. The essay should be on separate, numbered sheets of paper and include your name at the top of each sheet. The essay should be no more than 300 words.

    Public Health Essay

    Choose and describe a public health issue/problem in which you have an interest and present at least one possible approach to addressing that issue/problem. The essay should be no more than 1,000 words.

Notes

  • A complete, self-administered application package is required for admission to all programs. A complete application includes all of the items listed on the Application Cover Sheet (Step 3 above). The entire application is required from you in one envelope at one time.
  • Neatness and legibility will speed the processing of your application.
  • Type or print your name at the top of any separate sheet of paper enclosed with your application.
  • We rely on email as our primary means of communication with applicants.It is essential that you provide a clearly written, valid email account on your application form and keep us updated if you change your email and/or your postal mailing address
